A 34 year old woman with gestational diabetes had vaginal delivery at 34 weeks pregnancy. A male child weighing 3.2 Kg was born after a difficult and prolonged breech delivery. The baby was shifted to the intensive care due to tachypnea. On examination the baby was blue, heart rate 120/mt and respiratory rate was 60/mt. Subcutaneous emphysema was noted around the neck He was treated for ARDS.
