Case presentation ·         A 38-year-old male patient was referred to the hospital after outpatient labs revealed low hemoglobin. ·         A past medical history of acquired immunodeficiency syndrome and noncompliant antiretroviral therapy (ART) regimen was reported; the last CD4 count was unknown.

·         He also gave a one-week history of melanotic stools, generalized weakness, and progressively worsening shortness of breath. Vitals ·         Pulse – 106 beats per minute ·         Oxygen saturation – 98% ·         Blood pressure – 122/80 mmHg ·         Temperature - Afebrile
