A 49-year-old male patient with no history of diabetes, hypertension, smoking or alcohol use presented in the ER with severe chest pain of 45 mins duration, (retrosternal with no radiation), preceded by sudden profuse sweating. No h/o breathlessness ECG as shown in picture (leads V2-V6) Patients PR was 60 bpm and BP was 250/150 mm Hg. Systemic examination was within normal limits.
Patient was put on intravenous NTG to reduce his BP. What are the DDs and if MI is considered at what BP patient can be thrombolysed?
