I have seen so many "undiagnosed" fever cases in daily practice. Most of the time, you won't be able to prove the source of infection, especially in neurology patients. So if we are not getting positive culture results showing something (mostly) we are forced to try antibiotics because of highly demanding patients. Empirical treatment is the "rule" than exception because we need patients alive at that particular point in time, where guidelines take a backseat, I promise.

Apart from a theoretical discussion on these guidelines what's the practical solution for an antibiogram in a given situation, rather than talking behind the back of misuse of antibiotics? After all, why should we misuse, the patient is our priority. Please share your valuable opinion.
