I am treating a patient complaint of only anal itching since more than 1 year. All her investigations are normal except AEC-240, and have given her course of allegra 180 mg BD along with metrogyl and mebex in the due course. She is completely well for 15 days, but again developed the same. I re-examine her, small ulcer were seen near the anal opening but below the verge. I repeat metrogyl,and cefhexime 200 mg, cpm and syp looz along with dressing and ointment with clocip, clotrimoxazole and gtn 1%, Lignocaine gel 2%.She again had relief for 15 days, now she again has the same itching which has increased in intensity.
What should I do now. I have told her to get HBA1C,ESR,CBC, Thyroid profile along with Liver function test done and come to me after few days. Can u please suggest me where I am lacking? Kindly reform the treatment if needed. Please help.
