A 42-year-old tall man with long legs and arms was brought to the emergency department with severe chest pain, which was unrelieved by sub lingual nitroglycerin. He never had this symptoms before. He has no history of cardiac diseases in his family. He only takes enalapril for hypertension. He does not drink alcohol, smoke, or use illicit drug. He is physically active.

He had a new murmur of aortic insufficiency. The chest X-ray showed widened mediastinum.
