A 55-year-old woman is diagnosed with adenocystic carcinoma of right middle ear. She undergoes radical surgery which involves mastoidectomy with partial parotidectomy and radiotherapy. Four years later when she is evaluated for unrelated issues, peculiar facial features are noticed. She is unable to close her right eye completely or blow out air through her mouth. There is loss of wrinkling on the right side of forehead. Salivary flow was normal on both sides.
