Case presentation Presenting complaints: A 61-year-old male was presented to the emergency department (ED) with complaints of chronic cough, productive of white sputum, and occasional dyspnea on exertion. The patient  was non-responsive to the inhalation pump. Medical History: The patient was a non-smoker with a history of ulcerative colitis, dyslipidemia, and exercise-induced asthma. Laboratory Investigations:  Vital signs and physical examination were normal.

Six months old Spirometry results: Normal Forced vital capacity (FVC), Forced expiratory volume (FEV1), FEV1/FVC ratio, and diffusing capacity of the lungs for carbon monoxide (DLCO).
