Case presentation A 69-year-old lady with complaints of fever, hypotension, and altered consciousness was admitted to a hospital. The patient had type 2 diabetes mellitus, coronary artery disease, and hypertension. She was also chronically anemic and refractory to blood transfusions. Physical examination Diffuse purpura in the abdomen and bilateral lower and upper extremities Blood tests Other laboratory tests Less than 10% activity of von Willebrand factor-cleaving protease, ‘a disintegrin and metalloproteinase with a thrombospondin type 1 motif member 13 (ADAMTS13) was found.
