A 29-year-old female was brought into the accident and emergency department by ambulance after being thrown off her bike. She fell sideways and head first onto the ground. However, she was wearing a helmet during the fall. Paramedics reported her consciousness level dipped en route to the hospital. On assessment, the woman was unable to follow commands. Her eyes were closed and opened only when painful stimuli were administered above her eyes or sternum. She made mumbling noises that made no sense.

Her arms and legs withdrew away from the painful stimuli. Physical Examination:  The patient’s heart rate was 62 beats/min, and blood pressure was 148/90 mmHg. The respiratory rate was 18 breaths/min. The oxygen saturation was 99% on 4 L of oxygen. There were no palpable fractures over the skull, although there was a large hematoma above her right ear.
