Case presentation An 18-year-old nulliparous woman presented with a history of severe and worsening dysmenorrhea. She complained of cramps in the lower abdomen and irregular menstrual cycle with excessive bleeding since menarche at 14 years of age. Unfortunately, these symptoms were insufficiently relieved by NSAIDs and oral contraceptives. History No history of surgery, abortion, or any trauma Unremarkable family history Pelvic examination Normal vagina, vulva and adnexa A normal-sized anteverted uterus Transvaginal ultrasound Uterus of normal volume and irregular contours Homogenous myometrium and endometrium, without any change in thickness.
