Case Presentation A 36-year-old male presented to the ED with a 2-week history of headache, drowsiness, fever, and photophobia. The patient was a homosexual. He was previously healthy and sexually active.

Physical examination Temperature: 38.3°C Neck stiffness Radiograph analysis CT scan of the head was normal Other clinical examinations Opening pressure on lumbar puncture: 29 cm of water Cerebrospinal fluid (CSF) cell count: 340 cells/microliter (90% mononuclear cells, with the predominance of lymphocytes) Glucose level: 46 mg/deciliter Protein level: 0.80 g/liter Cryptococcal antigen titer: 1:128 Human immunodeficiency virus antibody: Positive Viral load: 300,000 copies/milliliter CD4+ count: 7 cells per/microliter ·CSF culture, gram staining, and India Ink stain results were awaited.
