A 10-year-old child was presented with acute onset fever and progressive worsening of respiratory distress for 1 wk. He had previously received intravenous antibiotics and oxygen by face mask for 3 days at another hospital but didn’t improve. He had tachypnea, chest indrawing, bilateral wheeze, and crackles. No asthma was noted in his family history.

However, he was treated for acute severe asthma with 3 cycles of inhaled bronchodilator, intravenous corticosteroid, magnesium sulfate, ketamine and terbutaline infusions as per standard protocol, but yet not improved the condition.
