Case presentation Mr. S, a 73-year-old man was admitted to the hospital for exploratory surgery for his abdominal pain. During the surgery, Mr. S experienced a rare phenomenon of accidental awareness during general anesthesia (AAGA). In this condition, Mr. S was able to feel all the pain of his body while being cut open; however, he could not express the pain or shout either.

After the surgery, the patient went into a state of psychological trauma. During this state, the patient complained of experiencing nightmares where he could perceive that ‘people were trying to bury him alive’ . After experiencing several events of hallucination, Mr. S shot himself to death 14 days after the surgery.
