I was called to see this middle aged lady admitted with fractured ankle 3 days ago with c/o orthopnea and vague tendernes R-chest in R-lateral position for the past 6 months. She has been afebrile throughout with minimal cough. Her CT chest revealed Posterior basal segmental consolidation. Her Echo showed moderately dilated RA & RV with mild PAH. She doesn't have any significant comorbidities.
Her INR & D Dimer is normal. I am planning for a pulmonary angiogram. Kindly give your thoughts on possible diagnosis & investigations.
