A 45-year-old woman presented with complaints of progressive muscle weakness and numbness along with a tingling sensation in her lower extremities. This later progressed to her upper extremities, six weeks post a complex surgery. Medical history The patient had a history of hypertension and morbid obesity for which she had undergone a sleeve gastrectomy, three months before the presentation. Past complaints The patient's symptoms began after a few days of recurrent emesis of unknown cause. This was followed by worsening episodes of inattention, disorientation, and staring spells.

After a few days of surgery, she was complaining of an absence of reflexes with a decreased muscle tone. CSF analysis  revealed an albuminocytologic dissociation.
