ECO 2025 delivered critical updates that are poised to reshape clinical approaches to obesity, endocrinology, and metabolic care. One of the most compelling sessions revisited the pathophysiology of obesity, presenting robust evidence in favor of the carbohydrate-insulin model —where hyperinsulinemia driven by processed carbohydrates promotes adipose tissue storage, suppresses metabolic rate, and triggers compensatory hyperphagia, challenging the longstanding energy balance paradigm. Clinically relevant insights also emerged on the use of epicardial fat thickness as a surrogate marker for visceral adiposity and cardiometabolic risk, and on the microbiome–adipose tissue axis, revealing microbial signatures that predict metabolic phenotypes beyond BMI.
