Case presentation : A 63-year-old woman visited an ophthalmologist, complaining about itching in her eyelids. She reported a history of burning sensation and pain in both her eyes. A month earlier, the woman visited a different hospital where she was misdiagnosed with an anterior blepharitis infection. The doctor prescribed Levofloxacin and corticosteroid eye drops, but the woman showed no signs of improvement.

Physical examination : Visual acuity test showed 20/20 of both eyes and intraocular pressure of 15 mmHg in the right eye and 12 mmHg in the left eye. A closer examination revealed light conjunctival congestion, normal cornea with no deformity in the fundus. Slit-lamp Exam : Microscopic examinations revealed more than ten crab-like lice.
