Case presentation A 28-year-old male with no significant past medical or family history presented to the emergency department six days after experiencing a sudden “pop” sensation in his head during lat pulldowns at the gym. At that moment, he had transient blindness in his left eye lasting 20 minutes, accompanied by nausea, and dizziness. As symptoms briefly improved, he was able to drive home.
However, over the next six days, he developed intermittent headaches, nausea, and vomiting. On the sixty day, he experienced persistent dizziness and intractable vomiting.
