Case presentation A 50-year-old lady was referred for a second opinion to a specialist practicing in the Department of Respiratory Medicine after experiencing recurrent dyspnea, fever, left-sided chest pain, dry cough, and pneumonia - on and off for a year. She also had repeated episodes of lung infection during the last year and was admitted and managed with antibiotics for pneumonia multiple times.

During that period, she was confirmed with - and managed for - atrial flutter. However, the symptoms remained unaffected.
