Davidsohn first described primary pulmonary leiomyosarcoma (PPL) in 1903 as an extremely rare and clinically aggressive form of malignant lung tumor that often goes undetected. It accounts for a total of <0.5% of pulmonary malignancies and often originates from the smooth muscle cells of the pulmonary interstitium, bronchial tree, or blood vessels. Unfortunately, the misdiagnosis or late diagnosis of the aggressive clinical profile leads to detrimental outcomes. Case presentation A 40-year-old Caucasian female with a history of morbid obesity, hypertension, infertility due to polycystic ovarian syndrome (PCOS), and nephrolithiasis visited the hospital for a check-up.
