1-year-old twin sisters 7.4 kg and 7.2 kg with normal antenatal, perinatal and developmental history till now is suffering from repeated seizures for 2 months. One had generalized, epileptic discharges so keppractice syrup was begun and later dose increased after 3rd seizure episode. In another twin sister too keepra syrup was added after 2nd episode of seizure. CBC, ABG, All profiles, urine calcium, CT scan of brain, BP results were normal. In 2nd such episode parents complained about very offensive several semi solids stools occurred in both the twins.
So IgA and anti-DPG IgA was done which came normal. But suspecting strongly celiac disease in both empirically gluten restricted diet for last 15 days is only going on and stool drastically improved. So HLA typing for it was asked and has come +very. So it's celiac associated GTCS in both twins. It's perhaps 1st reported case in India…
