USg- IHBRD with multiple stones in the Gall bladder.S he has not passed stool or flatus for the last two days. Abdominal examination reveals tenderness over the right upper quadrant, absent bowel sounds and no fecal matter in DRE. NG output is 600ml.
Total counts 21000, Neutrophils 85%, Bilirubin 20gm/dl conjugated. Blood pressure 80/50 mm Hg on vasopressors, tachycardia is present. Serum amylase and lipase is normal. What should be done in this case?
