Case presentation A 56-year-old Caucasian male had a fall from a staircase. He reported to the hospital six days later via the podiatrist with a history of a hot, swollen, and painful right foot, causing an inability to bear the weight for the last two days. The podiatrist had been treating him in the outpatient diabetic foot clinic regularly for his left first metatarsal head plantar neuropathic ulcer.

Following debridement and dressing, he was placed in a left total contact cast (TCC), which resulted in satisfactory ulcer healing.
