Cyclic Cushing’s syndrome remains one of the most challenging forms of hypercortisolism, with fluctuating cortisol peaks and troughs leading to delayed or missed diagnoses, unnecessary surgery, and worse remission rates compared with non‑cyclic disease. This clinical commentary highlights how heterogeneous biochemical criteria, variable sampling strategies, and inconsistent nomenclature hamper research, risk stratification, and therapeutic decision-making in this rare entity. The author advocates for internationally agreed definitions, standardized dynamic testing algorithms, and harmonized reporting of cyclic patterns to improve comparability of studies and, ultimately, outcomes for affected patients. To know more Click Here ##Reference## Niculescu DA.
