Case presentation A 58-year-old Caucasian woman reported to the emergency department (ED) with the chief complaint of left-sided chest pain and left shoulder pain for a year, which aggravated over the past three weeks . The pain was severe, frequent, and inconsistently worsened with changes in posture. In addition, it was associated with dyspnea, left-sided neck pain, and belching; without fever, chills, cough, abdominal pain, nausea, vomiting, diarrhea, dysphagia, melena, or hematochezia.

Medical history Diagnosed with stage IV colon cancer with liver metastasis since 2.5 years; and was undergoing treatment with FOLFOX, a combination chemotherapy regimen, and bevacizumab.
