A 55-year-old woman presented with a two-day history of fever and sore throat that had become worse, to the extent that she could no longer swallow her saliva. Her face was swollen and an inflamed, tender submandibular swelling had developed. On examination, she was sitting upright with her neck forwards and chin raised. Her breath was unpleasant, there was some trismus, the tongue was elevated by a brawny submandibular swelling, the tonsils were grossly enlarged and purulent, and her temperature was 38.5 ⁰C.
