Case presentation A 28-year-old pregnant female at 35 weeks of gestation presented to the emergency department with acute-onset severe breathlessness. She had a known diagnosis of pre-eclampsia and was on oral labetalol.

Her symptoms had worsened over a short period despite adherence to therapy.
