A 41-year-old woman presented to a General Physician with a history of dyspnea and cardiac arrhythmia for 5 months. A chest X-ray revealed the presence of an enlarged right mediastinum. Air content was present on the right side of her thoracic cavity. Thoracic CT scan revealed anteromedial diaphragmatic defect and herniation of the transverse colon.
