A 86 year old lady is complaining of a dry cough since 4 days which was - Sudden in onset and progressive increased in an episodic manner There were 2 major episode since 4 days Mark aggravated at night associated with chest pain during cough, breathing difficulty and difficulty in sleep Dyspnea on walking which was also present since 2-3 yr (previously able to walk around 100 meter, before cough complain) Frequency - whole night in episode No fever but mild headache present Sore throat and coryza present since 4 days History of same - 1 year ago and diagnosed as LRT other diseases - HTN and DM type 2 since 10 year Drug History - olmesartan 40, hydrochlorothiazide 12.5 and Amlodipine 5 mg, OD & Metformin ER 500 mg , glimepiride 2mg and Pioglitazone 15 mg.
