Long‑term follow‑up of a randomized trial of extremely preterm infants treated with low‑dose hydrocortisone for bronchopulmonary dysplasia prevention found no significant differences in global cognitive, motor, or behavioral outcomes at school age versus placebo. Importantly, rates of cerebral palsy and major neurodevelopmental impairment were similar, suggesting that this regimen does not carry substantial late neurotoxicity. These results help balance early respiratory benefits against long‑term safety concerns when considering hydrocortisone in the NICU.
