Case presentation A 28-year-old female was admitted to the intensive care unit (ICU) with a moderate traumatic brain injury. She was transferred to the medical unit under neurosurgery care. Her level of consciousness improved gradually but with periods of confusion and lethargy. In order to protect her airway as well as provide adequate nutrition, a small-bore feeding tube was inserted through her right nares. She received tube feeding at 60 mL/hour via a pump.

Her vital signs: Blood pressure (BP): 120/72 mm Hg; Heart rate (HR): 90 beats/minute (bpm); Respiratory rate (RR): 28 breaths/minute; Oxygen saturation: 96% on room air. Upon admission After two hours of admission, she was found with her feeding tube lying on her chest. She was restless, coughing, and struggled to catch her breath.
