Case Presentation: An Octogenarian man was diagnosed with benign prostate hypertrophy and bladder outflow obstruction with a large bladder stone. The patient underwent a cystoscopy and cystolitholapaxy. Surgical Procedures: An electrohydraulic lithotripsy was performed for disintegrating the bladder stone. However, this procedure was complicated by a perforation in the posterior bladder along with abdominal distention. A posterior injury in the bladder was noticed post-surgery, which was opened vertically and the perforation was repaired.

The procedure lasted more than 6 hours. The patient also developed sinus bradycardia (51 bpm). Post-Surgery Observations: The patient was stable after the surgery concluded.
