A 2025 analysis of the Children’s Oncology Group AHEP0731 Phase III trial evaluated the impact of comprehensive surgical guidelines on children with hepatoblastoma. Among 221 evaluable patients, 93% underwent definitive tumor resection , with 65% receiving surgery within the protocol-recommended timing . Nonadherence included early or delayed resections and liver transplants outside guideline recommendations . Despite this, event-free survival (EFS), overall survival (OS), and complication rates were similar between adherent and nonadherent groups.
The study demonstrates that structured surgical guidelines, coupled with chemotherapy and radiologic staging , increase resection rates and may prevent unnecessary liver transplantation in cases suitable for partial hepatectomy.
