A 2025 cohort of 39 post-neoadjuvant Whipple specimens shows that relying on gross tumor bed assessment alone significantly underestimates true residual disease. When the entire pancreas was submitted for microscopic evaluation, 62% of tumors increased in measured size and 28% were upstaged , largely due to microscopic foci, skip lesions, and intraductal cancerization that are invisible on gross examination. Lymph-node staging remained unchanged, underscoring that primary tumor dimension—not nodal status—is most vulnerable to underassessment after therapy. These findings highlight the importance of exhaustive sampling protocols in heavily fibrotic, post-treatment pancreata to ensure accurate ypT staging and guide adjuvant decisions.
