A 45 years old lady posted for abdominal hysterectomy. The pre-op evaluation revealed a k/c/o DM HTN Asthma. She was not on regular medications except inhaled steroids. Her investigations Hb 8gm/dl RBS 90mg/dl rest normal. 2 unit blood transfusion done. vitals BP 177/90 mm Hg PR 90 bpm. SpO 2 91 chest bil rhonchi. Echo concentric LVH EF normal. Spinal anesthesia with 25 G quinkes spinal needle and injection bupivacaine heavy 3.4 ml given.
Immediately after this pt. started labored breathing but vitals remains in limit BP around 150/100 PR 100 TO110 SpO 2 around 90 maintained. Pt was conscious but restless and unable to speak. After 10 to 15 min pt breathing becomes normal. Surgery started and done uneventfully. The postop period was also uneventful. Diaphragm movement was not there.
