Case presentation A 20-year-old woman, class 3, with a BMI of 43.4 kg/m 2 developed quadriparesis four months following laparoscopic sleeve gastrectomy. She never attended the prescribed post-surgery follow-up visits. She experienced numbness with tingling in the feet and both legs and weakness in the arm.

Soon after, the patient was unable to walk alone. Investigational findings Quadriparesis 3/5 in upper and limbs, absent deep tendon reflexes and negative Babinski sign Low-amplitude gaze-evoked nystagmus in left horizontal gaze Lumbar puncture: No abnormal findings  Metabolic work up: Folate 1.5 ng/mL, vitamin B12 874.7 pg/mL, ferritin 181.2 ng/mL, vitamin D 16.37 ng/ml, and thiamine 29.5 µg/L Electromyography indicated sensory polyneuropathy.
