As a practising clinical pathologist, I would like to share my experience reagrding interpretation of serum SGPT levels both in the methodology and the variables involved.The recommended IFCC method is as follows: L-Alanine +2-oxoglutarate ------ ALT/SGPT ------ Pyruvate +L- Glutamate Pyruvate +NADH --- LDH --- L -Lactate_NAD + L-alanine, 2-Oxoglutarate and LDH being an ingredients of the reagent supplied. The levels of SGPT is determined from the rate of NAD + generated at 340 nm, colorimetrically.
Now, the important variables affecting the reaction and consequently determination of SGPT levels are, Serum (sample)levels of SGPT Serum(sample) levels of NADH Serum(sample) levels of LDH All the above three variables are easily influenced by pathological and physiological causes such as heavy exercise, blood transfusion and extensive trauma other than ofcourse hepatic causes.
