In a large multicenter cohort study of 355 patients with type I congenital bile duct dilation and intrapancreatic bile duct involvement (I-IPBD) , radical extrahepatic cyst excision with intrapancreatic bile duct resection (REC) significantly outperformed subtotal excision (SEC). REC was associated with markedly lower rates of recurrent cholangitis, pancreatitis, and intrapancreatic bile duct stones, as well as reduced readmission, reoperation, and carcinogenesis rates, while achieving better quality-of-life scores. Importantly, perioperative complication rates—including severe and pancreas-related events—did not differ significantly between groups, underscoring REC’s safety profile. These findings support REC as the preferred surgical approach for I-IPBD.
