This 46 years diabetic man with body wt 110 kg, came to me with dyspnoea, cough, fever and restlessness. CBC 16300/cumm, X-ray chest on 16-2-18 showed only rt hilar region opacity. Given Antibiotics oral, then IV, oxygen, bronchodilators, expectorants, N-Acetylcysteine 600 mg 3 times a day. All treatment is given for 10 days with antidiabetic medication, (well controlled already). But this patient remained restless for 10 days. He says, he dysnoeanic, unable to take breath.
Look at repeat x-ray of 24-2-18 and suggest. TLC 13000/CUMM. Now, his wt is 100 kg, lost 10 kg in one month. Is it a carcinoma lung or Koch's chest?
