The National Medical Commission ( NMC ) has advised registered medical practitioners (RMP) to maintain the medical records of patients for three years from the last date of treatment . Maintaining medical records is listed as one of the primary responsibilities of RMPs in the Registered Medical Practitioners ( Professional Conduct ) Regulations 2022 draft . The duties of RMPs include digitalizing the medical records and delivering honest certificates and other documents on request.

The patient records should include age, gender, address, profession, dates of observations and the last visit, diagnosis, follow-up, and summary of the case, along with the signature and name of the physician.
