A 57/M c/o pain and unable to move right upper hand since 2-3 days back. No history of trauma or fall. Symptoms starts when he awoke at morning. With this much history he visited to orthopaedic. O/E patient has pain on abduction around deltoid range mainly. Orthopaedic surgeon prescribed NSAIDS & anti inflammatory drugs. On Follow up pain was reduced and range of movement improved but not completely. After 3 months of this treatment and some physiotherapy.
Recently 2 days back patient suddenly observed thinning & shrinking of muscle bulk of affected arm. O/E atrophy seen at region of long head of biceps and medial border of scapula of affected side. MRI SHOULDER s/o MINIMAL GLENOHUMERAL JOINT EFFUSION WITH FLUID ALONG TENDON OF LONG HEAD OF BICEPS MUSCLES AND ALONG SUBSCAPULARIS TENDON. Rest all normal.
