23 yrs old primigravida patient with 31 WOG with h/0 asthma for past 7 years. She was intubated due to poor gcs and respiratory distress She is on P-SIMV mode of ventilation.
Her spo2 was 93-95% Now deteriorated to 85-91% even at 100% FiO2 Her bld gas showed metabolic acidosis with pH of 7.18 and bicarb 5, co2 14, saturation 91% po2 100 Bicarbonate total 200 meq given. Her HB is 7 and INR is 1.9 so 2bt and 2 ffp transfused Her potassium on admission was 5.7 so calcium 1gm given Urine output 200 ml over 7 hrs not able to ascertain cause of desaturation How to manage this case?
