Osteoid osteoma (OO) is a rare benign bone tumor that usually affects long bones, but exceptionally can involve the temporomandibular joint (TMJ) , making diagnosis challenging. A 27-year-old male with a four-year history of left-sided TMJ pain was ultimately diagnosed with intraosseous OO through multimodal imaging and histopathology . Surgical excision led to complete symptom resolution, highlighting the importance of systematic diagnostic evaluation in atypical TMJ pain . This case emphasizes that persistent joint pain may occasionally mask rare osseous neoplasms rather than common temporomandibular disorders.
