Case presentation A 49-year-old female was admitted to the emergency department with complaints of anorectal pain and fever. Medical history Anorectal pain for three months Fever for one day Circumferential 4th-degree hemorrhoids Underwent double-purse string stapled hemorrhoidopexy three months ago Complained of perianal discomfort and difficulties in defecation six weeks after the operation MRI examination: Presence of rectal ring cyst Underwent incision and drainage through the edge of the anus Complained of persistent symptoms Endoanal ultrasonography: Clear fluid containing a cystic lesion circuit to the rectum and the rectal wall surrounding the cyst (Figure 1A) Subsequently underwent puncture and drainage through the vagina.
