A 2 year old previously normal boy is having recurrent tyhoid and paratyphoid infection for last 6 months. He was first infected in May 2015 and typhi IgM was +ve. Was treated successfully for 10 days and did well for 1 and 1/2 month,/then again fever was diagnosed as Tyhoid by IgM + DOt test and was treated for 2 weeks successfully.
To my dismay this month 3rd time he developed fever and Typhi IgM again came + ve.In none episodes IgG was +ve.This moment i did widal and found S.Typhi H and Paratyphi A +ve . I am thus surprised why IgG didnt came +ve even during 3rd episode as is ususal course after any infection at least for few weeks to months.Why IgG antidody didnt develop in this child even after 3 episodes of typhoid.This is the burning question? Or was it relapse of the same episode Of typhoid.Which i feel is not possible as i havent seen relapse in any child if treated…
