Case presentation A 44-year-old woman presented with a 1-day history of constant abdominal pain and vomiting after eating her evening meal. She also had intermittent episodes of bilious vomiting. She is a diabetic patient and is worried that her blood sugar levels might have increased as she has not eaten a normal diet since the pain started. Her bowels have opened normally, and there are no urinary symptoms. Examination The patient is febrile: 38°C, pulse rate: 116/min.

The right upper quadrant of the abdomen was tender, and it worsened after two fingers were placed beneath the tip of the ninth costal cartilage during inspiration. Tender mass is palpable in the right upper quadrant. Urine appears normal, and rectal examination is normal.
