A 40 years old male patient reported of enteric fever and treated for malaria a year back, reported with severe intermittend headache. The head ache is so severe that he would wake up half screaming at night. He get relief by pain killers and Inj. Diclofenac but has been unable to get much relief with injectable Diclofenac too. I had conducted test CBC - WNL, LFT, KFT, NAD, Electrolytes Normal, Chest XRay Normal, Lipids normal, Chest X Ray normal and CECT Head Normal. He still has intermittent fever and hadache.

He is currently on Aceclofenac, Tamadol, Vasograin, Multivitamins and Oral Cefixime/Azithromycin. What is your differential diagnosis? What would you suggest management of this patient. Thank you for your invaluable comments.
