For patients with proliferative diabetic retinopathy (PDR) undergoing pars plana vitrectomy (PPV) , the use of intravitreal anti-VEGF agents before surgery has proven to be a game-changer. In this systematic review and meta-analysis of 91 randomized controlled trials involving 8,721 eyes , adjunctive anti-VEGF therapy led to less intraoperative bleeding, shorter surgical times, and fewer complications such as retinal breaks, endodiathermy, and postoperative vitreous hemorrhage. Patients also achieved better visual acuity, improved retinal reattachment, and lower reoperation rates compared to PPV alone.
