As the treatment for metastatic urothelial carcinoma (mUC) is undergoing rapid evolution, the guidelines for treating the same are constantly updating. In light of the updated guideline of 2021 by the European Association of Urology (EAU), this article gives an overview of the recommendations for first-line therapy to treat mUC. The aim is to help clinicians and patients with an optimal treatment plan. 1.

First-line systemic therapy for mUC Untreated patients for mUC can be divided into three categories: Fit for cisplatin-based chemotherapy Fit for carboplatin-based chemotherapy (but unfit for cisplatin) Unfit for any platinum-based chemotherapy Figure 1 discusses the eligibility and ineligibility criteria of the therapies among the mUC patients.
